Other real estate owned (include
address, type, value, income, expense, and lease agreement).

Retirement fund value (include
the most recent statement, all pages).

Bankruptcy filing and discharge (all pages) if within the last
4 years.

Divorce Decree / Settlement Agreement / Property Settlement,
if applicable.

Verification of other income
(ie. dividends, alimony, child support, etc.)

Name, address and phone number
of landlord, if renting now / previously within the last 2
years.

Name and phone number of the
human resource department of current / past employers last 2
years.

Check for credit report and
appraisal if applicable.
